JOB PURPOSE: To ensure that patients care received and delivered in a timely
and efficient manner, and is appropriate to the correct care delivery setting
vs. the severity of illness of the patient. Also, to identify, assess, and
provide for possible post-discharge needs that may cause hospital mandatory over
stays.
MINIMUM QUALIFICATIONS:
Registered nurse/Social Worker in the state of West Virginia.
Minimum four (4) years hospital/clinical experience.
Working knowledge of approved criteria used for assessment/decision making.
Willingness and ability to teach others.
