Provides support to families and will be instrumental in monitoring the health and development of children enrolled in the Healthy Steps Program.  Position emphasizes a close relationship between the healthcare provider and parents by addressing the physical and emotional development, as a liaison between the provider and specialists.   Responsibilities will include meeting with parents of children at every well child visit from birth – age 3, holding support groups for parents on a regular basis, scoring ASQs, handling referrals to specialists/other resources for all providers treating pediatrics, educating parents on developmental matters as well as normal milestones for all ages/stages of growth, offering and providing home visits to enrolled families 1-2 times per year, post-partum screenings, working closely with community resources available to parents of young children, will also be responsible assisting with the Reach out and Read program by making sure all children receive a book during their Well Child Checks. Position may also provide assistance to all providers of CPMG in follow up of referral(s) made to outside providers for patients of all ages.  This may include assisting with obtaining durable medical equipment, medical supplies, oxygen, CPAP, etc.
Grant funded position.

Job Requirements

Requires active, current CO RN or LPN license and/orBachelor’s Degree; minimum of 2 years background in child development; validColorado Driver’s License and current auto insurance documentation that meetsColorado’s minimum requirements;  excellent verbal and writtencommunication skills; strong organizational skills; ability to workindependently as well as with multiple team members; ability to maintainconfidentiality; ability to prepare and maintain records accurately; ability toproperly document in charts regarding visits/conversations with families; ability to be self-motivated/directed and work with minimal supervision;ability to multi-task and be flexible; ability to establish priorities; abilityto meet deadlines; effective skills in identifying problems, usingproblem-solving methodologies, and recommending solutions; ability to completecomplex tasks in a timely manner; ability to think critically; ability to workwell under stressful conditions with time constraints; and knowledge of/abilityto gain knowledge of organizational policies, regulations, and procedures.Bilingual preferred.
