Hospitalist--Advanced Practice Provider Career Opportunity at Conemaugh Nason Medical Center


With direction and guidance from the designated Hospitalist Program Physicians, the Advanced Practice Provider will be responsible for the provision of care in a predominately acute setting.  The APP will also function in a versatile capacity providing multi-skilled services within the Medical Center, including within the organization's various clinics. 


Responsibilities include, but are not limited to the following:

-With the guidance and direction of the designated Hospitalist Program Physician, treats and provides care to hospital inpatients, including conducting patient evaluations and reviewing medical history.

-Prescribes medications and/or treatments to patients based on medical and health conditions.  Works with acting Physicians for directions as needed.

-Works with designated Physicians to order and review the results of patient tests/exams, including those from the Lab and Radiology (i.e. CT scans, MRIs, lab tests, etc.)

-Participates in the discharge planning as well as the discharge of Medical Center patients.

-Plays an active role in referring patients to medical specialists, social services, or other medical professionals as needed.

-Attends inpatient consultations in areas of specialty.  Documents and writes patient discharge summaries and send information to PCPs.

-Actively communicates with patients' PCPs upon admission, if treatment should change, or at time of discharge.

-Provides follow up and health maintenance care including the appropriate adjustment of medications in accordance with established protocols or in response to specific instructions from attending physician.

-Participates in required continuing education activities to maintain or enhance knowledge and skills. 

-Ensures consistent communication with Hospitalist Program Physicians in order to maintain continuity and quality of care.

-Adapts and assists in other key areas of the Medical Center, including the facility's Clinics as needed.

-Must have a working knowledge of electronic medical record (EMR) systems. 


Job Requirements

Minimum Education:

Must have successfully completed and graduated from an approved program for Certified Registered Nurse Practitioners or Physician Assistants.

Required Licenses:

Pennsylvania--Advanced Practice RN or Physician Assistant

-Must hold a valid PA CRNP License or PA Medical Physician Assistant License

-Must hold a valid Controlled Substance Certificate issued by the DEA

-BLS, ACLS, and PALS required

Skills:

-Must complete the full APP credentialing process

-Active listening, critical thinking, sound judgment and decision making

-Experience using EMR systems

Minimum Work Experience:

1-2 years of experience working as a CRNP or PA, preferably in an acute care/hospital setting

Position is weekdays, core daylight hours.
