Under the general direction of the Oncology Service Line Director and Oncology Service Line Clinical Manager the Lung Coordinator manages the daily operations and workflow of the Lung Cancer Screening Program and the Lung Nodule Program.  The Coordinator will be responsible for ensuring follow-up exams are performed on Lung Cancer Screening patients.  The Coordinator manage lung nodules and other incidental findings identified through various avenues including, but not limited to high risk lung screenings and incidental findings.  The Lung Coordinator will coordinate through Nurse Navigation the next level of care for the patients with lung nodules and assist patients, clinicians and other program stakeholders in the collection, reporting and presentation of program outcomes.

  • Work to resolve the identified patients for follow up, update and document in the software and communicate the same to ordering providers

  • Maintain open communication with all ordering providers (PCP’s, ARNP’s, PA’s, Pulmonologist, etc.

  • Identify RAD 4 patients for Tumor Board presentation

  • Ability to access, plan, intervene and evaluate the physical, emotional and educational needs of patients.

  • Ability to anticipate change and react accordingly

  • Ability to work efficiently both independently and as a member of a multidisciplinary team.

2) Clinical Responsibilities

  • Provides patients access to educational materials enabling them to make informed decisions

  • Participates in patient education by assisting patients and families in understanding their diagnosis, treatment options and available resources

  • Facilitates scheduling of appointments for clinical consultation and support services inclusive of the medical care team, surgeons, radiation oncologists, social workers, pain consults and dieticians.

  • Identifies new and established patient with a cancer diagnosis who require navigator services.

  • Matches any unmet need with appropriate referrals within the multidisciplinary team, including but not limited to, social work, dietitian, genetic counselor and pastoral care.

  • Serves as a patient advocate on the multidisciplinary team.

  • Transitions patients from one phase of case to the next, including self-management and independence in self-care.

  • Review orders prior to scheduling to ensure guidelines are met for exam.   

3) Leadership and Quality

  • Adheres to established productivity benchmarks and submits data for the oncology dashboard

  • Articulates service and customer relations improvements to ensure patient retention

  • Coordinates clinical information for multidisciplinary treatment planning cancer conferences

  • Develops patient education programs and written communication material

  • Participates in collecting data for the Cancer Committee to monitor quality metrics and clinical outcomes

  • Participates in Oncology Service Line performance improvement activities

  • Maintains responsibility for outreach efforts to establish and maintain positive working relationships with key customers and community (providers, office staff, diagnostic laboratory staff, nurses, radiology staff and social work

Job Requirements

ADN required; BSN preferred

Minimum two years of clinicalor non-clinical healthcare related experience.

Experience in Oncology preferred
