Job Summary:


The Patient Navigator provides navigation services across the cancer continuum to patients with identified barriers. Navigation services may include identifying and addressing barriers, and connecting patients to valuable resources both in-house and the community. Patient Navigator services are focused on identified concrete barriers to care (transportation, lodging, system barriers etc.) and are provided as needed throughout the care continuum. Additionally, the patient navigator functions collaboratively within the multidisciplinary team in advocating for, educating, and supporting oncology patients and survivors.

The patient navigator assists in the collection of outcomes data, in building and maintaining community relationships, and in connecting patients/families to resources to deliver a seamless model of access and care benefiting patients, participating physicians, and family members.

RequiredSkills:


  1. At least three years of experience withoncology patients in hospital, clinic or community setting preferred.

  2. Excellent written, oral and conversationalcommunication skills to effectively work with diverse groups.

  3. Ability to analyze, evaluate multiplesolutions and solve complex problems using well developed critical &analytical thinking skills. 

  4. Excellent time management skills needed toprioritize among many competing priorities. 

  5. Able to complete documentation in a quick andefficient manner.

  6. Attention to detail needed to operateaccurately and effectively.


RequiredLicense/Certifications:


  • RN,LPN, BSW, MSW, or Reg. Radiology Technologist licensure/certification

  • Bachelor’sdegree preferred

BLS certificationrequired, or obtained during first week of hire.
