Provides primary or acute healthcare services for certain acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice. Follows established policies and procedures, objectives, safety standards, and sensitivity to confidential information. Provides primary or specialty medical care with the supervision of a physician in accordance with best practice. Take health histories and performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician. Assess the health status, illness conditions, response to illness, and health risks of the patients. Assess strengths, weaknesses, resources, coping behaviors, and the environment of the patients. Discusses and formulates proper and personalized health care plans for patients in consultation with the doctors, specialists, and other medical professionals. Evaluate patient treatment plans and modify such plans where needed. Records physical findings in the Electronic Medical Records (EMR) system, and formulates plan and prognosis, based on patient's condition. Under the delegated prescriptive authority of the physician, prescribes medication from the formulary, and consult, when needed, for those medications not approved in the formulary. Makes referrals for patient to physician or other specialized health resources for treatment. Consults physician when necessary including, patients who require further referral or hospitalization; patients who do not show a clear diagnosis; patients who do not show signs of improvement on the current treatment plan; or other circumstances determined to be unusual, unique or problematic. Provides appropriate documentation for physician review. Provides adequate documentation to support billing and collection services including, coding, prior authorization, and pre-certification. Answers any queries from patients about medical conditions. Maintains confidentiality in accordance with HIPAA regulations. Support the culture of service excellence throughout the organization. Actively seek ways to demonstrate a commitment to continuous improvement and participate in professional development opportunities. Performs other related duties as assigned or requested. Completes annual educational requirements. Understands and adheres to compliance standards as they appear in LifePoint's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Computer and/or keyboarding skills required.

Job Requirements

Minimum Education
Masters Degree from a Physicians Assistant program.
Certification from the NCCAP.
