Wilson Medical Center
An active member of the interdisciplinary healthcare team the Case Manager facilitates internal and external delivery of healthcare services. The Case Manager integrates the roles of Utilization Management, Case Management and Discharge Planning. The Case Manager is responsible for care coordination and development of the discharge plan through collaboration with the healthcare team and internal/external resources. The job objective is to promote a collaborative interdisciplinary care delivery model to achieve optimal clinical and financial outcomes with adherence to regulations and compliance.

This position is weekends: Friday, Saturday and Sunday, 12 hour shifts.

Job Requirements

Required Education: 

  • Associate’s degree Required
  • Bachelor’s degree   Preferred  

Required License/Certifications:

  • Current/active North Carolina or compact RN license

Required experience:
  • Three years of experience in one of the following areas:  Case Management, Utilization Review, Discharge planning or
  • Five years of med/surg experience

 Required Skills:

  • Ability to understand, communicate, and reacteffectively to patient and family needs

  • Critical thinking

  • Work well independently while keeping calm andmaintaining customer focus

  • Must be able to work collaboratively withinternal and external resources for optimal discharge planning
