Job Description

UP Health System - Marquette
Works as an intermediary between the on-call physicians and the referring physicians in the community setting to meet the placement and treatment needs of the patient.
Collaborates with on-call physicians to provide a seamless transition of care for the patient being received from outside facilities/providers.
Collaborates with House Supervisor/Bed Control on placement of patient in the appropriate care setting without the referring entity experiencing delay.
Maintains communication with outlying referral sources regarding patient status.
Completes follow-up on discharges from the hospital and Emergency Department.
Works as an advocate for the physical and emotional well-being of the patient.

Job Requirements

Minimum Education
  • Graduated from LPN, LVN program required
Required Skills
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Certifications
  • Basic Life Support Health Care Provider (BLS-HCP) required
Licenses
  • Licensed Practical Nurse (LPN) required
Minimum Work Experience
  • 1 year hospital experience required
  • 3 years acute care experience preferred
